The Massachusetts Avenue Surgery Center will make every effort to make your stay with us as comfortable as possible. To expedite your check in process, we have developed a series of commonly used forms which you may complete in advance and fax to the Center. If you choose to fax these forms, they must be received by the Center no less than three days prior to your scheduled procedure. Fax them to us at 301-263-0820.
